Hazard-identification Systems
Helmet Fire, Inc., has successfully fielded (and fully supported) reporting systems for a variety of fast-paced, high-risk, little-margin-for-error domains. Examples include:
- Emergency Medicine
- Physicians-in-Training (EM Residents)
- US Navy Helicopter Squadrons (Air Ambulance Detachment)
- US Navy Submarine Crews
We have also developed, but not yet fielded, a Medical
Transport variant that could be easily adapted to the EMS domain.

Medical Human Factors
Rollin J. (Terry) Fairbanks, MD, MS, FACEP is Assistant Professor of Emergency Medicine at the University of Rochester School of Medicine. He is a patient safety/Human Factors Engineering researcher, EMS Medical Director, and board certified emergency physician. His clinical practice is in the adult and pediatric emergency departments at the University of Rochester Medical Center/Strong Memorial Hospital in Rochester, NY.
Dr. Fairbanks has made numerous presentations on event reporting systems and their potential to contribute to systems-based improvements in patient safety.
